Please refer to Page 2 of the TC-40 Utah Tax Return Form, Line 19.
Fill in CODE 05 (School District & Nonprofit School Dist. Foundation), dollar amount, School Dist. Code 14 (Jordan). Your refund will be reduced by this amount. If you owe, your tax amount will increase by this amount.
